If you are planing to visit one of our offices, it is beneficial to have medical records sent to The Fertility Institutes in advance. This can be done by requesting that your records from other providers you may have seen be sent to us by mail or we also give you the option of filling out your patient registration and medical histories in advance as follows:

Financial Planing
We offer patient assistance with financial planing of fertility services. Loans are available for all qualified U.S. and most Canadian applicants.
Insurance Options
We offer patient assistance obtaining HMO and PPOÂ insurance benefits. We are not "providers" but have obtained patient benefits in selected cases.
