Patient Registration

If you are planing to visit one of our offices, it is beneficial to have medical records sent to The Fertility Institutes in advance. This can be done by requesting that your records from other providers you may have seen be sent to us by mail or we also give you the option of filling out your patient registration and medical histories in advance as follows:
